Continuous Infusion Versus Intermittent Bolus Administration of Meropenem in Severe Sepsis: A Multicenter Randomized Controlled Trial

A randomized controlled trial (n=412) demonstrating enhanced microbiological cure and reduced length of ICU stay with continuous infusion meropenem in septic shock.
Abstract
Background: Time-dependent beta-lactam antibiotics require plasma concentrations exceeding the MIC (fT>MIC) for optimal bactericidal activity. This multicenter RCT evaluated whether continuous infusion of meropenem improves clinical resolution and decreases 28-day mortality in critically ill septic patients compared to standard intermittent dosing.
Results: Continuous infusion achieved 98.2% pharmacokinetic target attainment vs 74.5% in the intermittent arm, with a 6.2% absolute reduction in ICU mortality.
